Medical Records
To request medical records you will need to contact a release of information associate between the hours of 8:00am and 4:00pm, Monday through Friday, at 530-244-5300. Associates will be able to assist you with the request process. By calling ahead and speaking to a release of information associate, you may avoid having to wait while records are copied and/or printed, or avoid an unnecessary trip to our hospital.
You can obtain an authorization form from Shasta Regional Medical Center HIM departments, or by clicking on the Release of Medical Information/Authorization Form for a printable form. You can bring in your completed form to Shasta Regional Medical Center 1100 Butte Street, Redding, CA 96003. Please call 530-244-5300 if you have any questions.
The Medical Records Department (Health Information Management) has the right to process your request for your medical records within a reasonable amount of time. Your request will be processed within five (5) business days and mailed to you. If your healthcare clinician needs the records right away for an appointment, the Health Information Department will fax your records to your care clinician within 24 hours.
If you have an emergent need, please contact the Health Information Department.
Records needed for continuing care purposes will be sent directly to the clinician eliminating the need for you to make a trip to Shasta Regional Medical Center. Requests for medical records for reasons other than continuing care, or for someone other than yourself, must be processed in person. We require a signed authorization and a valid photo ID. If you request a copy of your record for personal use there is a nominal charge.
